Patient Check-In Protocol

In order to reduce personal contact and increase social distance, LSMC will implement the following procedure for any medical or physical therapy appointments, in this office.

1. All patient information will be taken by the Front Office Staff prior to appointment by telephone. We will no longer be completing patient registration forms.
2. Upon arriving at LSMC’s parking lot, STAY IN YOUR VEHICLE and
   dial:(956) 415-0222, in order to check-in for your appointment.
3. A Nurse will call you to conduct an over-the-phone based triage.
4. Upon completing your over-the-phone triage, a Nurse will meet the patient at the door.
